We are seeking a dedicated RN Care Manager for a leading healthcare provider in Roanoke, VA, to bring comfort, compassion, and quality care to patients and families when they need it most. This contract position focuses on case management, requiring substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education.

As an RN Care Manager, your responsibilities will include:

• Managing patient care and providing services with a focus on quality and compassion.
• Utilizing excellent written and verbal communication skills to educate patients and families.
• Demonstrating problem-solving and decision-making abilities in a dynamic environment.
• Developing and maintaining rapport with patients and families.
• Collaborating effectively with an interdisciplinary team.

Working Conditions:

• Primarily works in patients' residences.
• Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary.
• Overnight travel outside of the service area may be necessary for continuing education.
• Employee is at direct risk for exposure to infections and bloodborne pathogens.
