Description

American Traveler is hiring an experienced RN Care Manager for an acute care case management role requiring 2+ years of RN experience and NC or compact licensure.

Details

• Hospital-based Care Management department serving a 20-25 bed unit
• Patient population includes Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown services
• Schedule is Monday through Friday, 8:30am-5:00pm
• No weekends, holidays, or on-call required
• Uses Epic EMR for documentation and care planning

Requirements

• Active RN license in North Carolina (Standard or Compact)
• Minimum two years of RN healthcare experience
• Prior case/care management or discharge planning experience in an acute care hospital setting preferred
• Nursing Diploma, ADN, or ASN required; BSN or MSN preferred
• Strong assessment and critical thinking skills
• Epic EMR experience preferred

Additional Information

• Provide comprehensive assessment, planning, implementation, and evaluation of individual patient needs
• Coordinate with multidisciplinary teams including Social Work, Utilization Management, and Care Management Assistants
• Participate in daily Care Management Touchpoints, CAPP meetings, and weekly Complex Care Meetings
• Identify barriers to discharge and coordinate post-acute care referrals and authorizations
• Experience in teaching or academic hospital settings is helpful
• Highly organized with strong attention to detail and adaptability to frequent change required
