Description

American Traveler is hiring an RN Care Coordinator for a Case Management/Utilization Review role requiring an active MA RN license and Epic charting experience.

Details

  • Hospital-based Care Coordination/Case Management (CCM) nursing unit
  • Manages a caseload of patients across the episode of illness from pre-admission to post-discharge
  • Day shift schedule, 07:00-15:30
  • Uses Epic for charting

Requirements

  • Active MA RN license required; conditional or pending licenses not accepted
  • AHA-issued certifications only
  • Primary source verification of license/certification required within 30 days of start, updated upon expiration
  • Both NURSYS and BON verification required for nursing license
  • Epic charting experience required

Additional Information

  • Coordinates care prescribed by an interdisciplinary team utilizing patient assessment, care guidelines, protocols, and payer regulations
  • Participates in ongoing evaluation of practice patterns and systems to support quality, cost, and satisfaction outcomes
  • Mobilizes resources to maximize efficiency of care delivery
  • Flu vaccine required with no religious or medical exemptions allowed
  • Local candidates must reside within 60 miles of Massachusetts General Hospital in Boston
  • No validated parking available at Boston locations; public transportation or paid public parking required
