Job Description

The Registered Nurse (RN) plays a crucial role in delivering high-quality rehabilitation care. This position is responsible for maintaining and following the individualized nursing care plan for patients while supervising Licensed Practical Nurses, Certified Nursing Assistants, and new staff.

Responsibilities

• Conduct nursing assessments on all patients within 8 hours of admission and utilize data collected to form a patient-specific care plan.
• Identify teaching needs and set related goals through collaboration with patients, families, and interdisciplinary teams.
• Provide timely updates on patient information for Initial Team and Re-evaluation Conferences, ensuring accurate documentation of assessments and interventions.
• Participate actively in the rehabilitation program on an assigned unit with an emphasis on patient education based on health needs.
• Initiate discharge planning and patient teaching from the day of admission.
• Communicate significant changes in patient status to the care team to support continuity of care and patient safety.
• Supervise, train, and direct ancillary nursing personnel, fostering an efficient and positive work environment.
• Promote personal and environmental safety by adhering to hospital policies and safety procedures.
• Demonstrate leadership by maintaining effective communication with management regarding service needs.
• Assume charge responsibilities as needed, prioritizing patient care and staff collaboration.

Required Experience

• Minimum of two (2) years of bedside clinical experience required.
• PCU/Critical Care experience is highly desired.

Certifications/Licensure

• BLS Certification required before the first shift.
• ACLS certification required before the first shift (CIRH only).
