Responsibilities
- Deliver coordinated nursing care for an assigned patient group in the home hospice setting, following established nursing processes.
- Conduct data collection and assessment, analyze findings, and develop and implement care plans to achieve expected outcomes.
- Administer medications per orders, monitor for adverse effects, and document care in the electronic health record; use telehealth tools to complement in-person visits as appropriate.
- Educate patients and family members about disease management, symptom control, medication use, and available hospice services; participate in discharge planning to ensure continuity of care.
- Coordinate with multidisciplinary team members (physicians, social workers, therapists) to promote safety, comfort, and quality of life; delegate appropriately and supervise assigned nursing staff.
- Adapt care delivery to changing patient needs, maintain a safe home environment, and provide learning opportunities for patients, families, and team members.
Requirements
- Active Registered Nurse (RN) license in the state of practice (California) or state of assignment
- BSN degree preferred
- At least two years of recent RN experience in the field
- Current BLS/CPR certification; ACLS certification required
- HLFS (Hospital Fire and Life Safety) certification required
- Ability to work in a home healthcare – hospice setting and to fulfill night shift requirements
