Responsibilities
- Conduct 25–30 patient visits per week, including a mix of start-of-care and revisits
- Perform and document OASIS assessments for eligible visits
- Utilize Epic for charting, care planning, and documentation
- Assess patient needs, develop and update individualized care plans, and coordinate with the interdisciplinary team
- Provide patient and caregiver education, promote safety, identify and resolve potential issues, and ensure timely follow-up
- Communicate clearly with patients, families, and supervisors to maintain quality care and accurate records
Requirements
- Minimum 2+ years of recent and relevant home health nursing experience
- Proficiency with OASIS documentation and Epic electronic medical records
- Ability to manage 25–30 visits per week with a mix of start-of-care and revisits
- Daytime schedule availability and willingness to travel within the service area
- Commitment to a 12-week travel assignment with guaranteed hours
- Strong clinical assessment, documentation, and communication skills
