    Overview
    Registered Nurse, Case Management in Saint Helena, CA, United States. This is a travel - temporary assignment starting 08/24/2026 for 13 weeks. Compensation ranges from $2,316 to $2,361 per week, with guaranteed hours of 36 per week. Shift is day shift (7:00 AM–7:00 PM). Work setting focuses on in-home care through home health and hospice services, with a focus on coordinating patient care and guiding care teams in the patient’s home.

    Responsibilities
    - Complete initial and ongoing comprehensive nursing assessments for patients in the home setting; document patient status and care milestones.
    - Develop and update the plan of care based on identified needs, goals, and input from the patient, family, and care team.
    - Provide direct and/or indirect skilled nursing care, coordinate with other disciplines, and ensure timely communication of patient status and changes.
    - Educate patients and families about nursing care, treatment plans, medications, safety, and patient rights; provide written materials as appropriate.
    - Identify problems or abnormal findings requiring follow-up; adjust plans of care and medication profiles as changes occur.
    - Facilitate goal clarification with patients and families, coordinate with the care team to meet care goals, and ensure documentation complies with policies and regulatory standards.

    Requirements
    - Licensure: Registered Nurse (RN) license in the state of practice (California)
    - Certifications: CPR or Basic Life Support (BLS or AH-approved equivalent) required
    - Experience: At least two years of recent clinical experience in the field; acute care or home health experience preferred
    - Education: Bachelor’s Degree in Nursing (BSN) preferred
    - Additional: Valid driver’s license, current automobile insurance, reliable vehicle; must be at least 21 years old
    - Skills: Ability to perform comprehensive home health assessments, coordinate multidisciplinary care, and document accurately and timely; effective communication with patients, families, and care teams
