Responsibilities
- Conduct initial assessments, develop care plans, and coordinate case management for identified patient populations
- Facilitate interdisciplinary collaboration to optimize care, monitor utilization, and support appropriate resource use
- Plan and coordinate patient transitions, discharge planning, and referrals to post-acute services
- Maintain accurate documentation in the electronic health record and monitor care quality metrics
- Provide patient and family education, advocate for patient goals, and ensure compliance with regulatory standards
- Float to sister campuses as needed; adhere to the 50-mile radius rule; respect unit culture and workflows
- Complete required prescreening steps and participate in interview and offer processes as outlined
Requirements
- Active Registered Nurse license in Florida; Nursys must be on profile
- BLS certification
- 2+ years of Case Management experience
- Care Management certification (AMC or CCM) preferred
- Degree accreditation must be on profile
- Ability to work in a hospital setting and float to sister campuses as needed
- All demographic information must be loaded at submission (address, email, full DOB, full SS, RN license number)
- RTO requests must be submitted with the submission; no post-submission approvals
