Responsibilities
- Conduct comprehensive case management assessments for hospitalized patients to identify discharge needs, risks, and barriers to safe transition.
- Develop and coordinate individualized care plans in collaboration with the interdisciplinary team; arrange post-acute services and community resources as appropriate.
- Facilitate timely discharge planning and patient education, ensuring clear communication with patients, families, and care teams.
- Monitor patient progress, document utilization and outcomes, and support adherence to payer guidelines and hospital policies.
- Collaborate with physicians, social workers, and ancillary staff to optimize care with an emphasis on reducing readmissions.
- Participate in readmission reduction initiatives and support float as needed within the unit and related campuses.
Requirements
- Florida registered nurse license; FL/Compact license required (recent Nursys must be on profile)
- BLS certification
- 2+ years of case management experience in an acute care setting
- Degree accreditation must be on profile or must have 5 years of case management experience
- Ability to work in a hospital environment and participate in floating to other units or campuses as needed
- Prior travel experience is preferred
