Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 2+ years of recent experience and OASIS proficiency.

Details

  • Home Health setting with a geriatric patient population
  • Caseload of approximately 5-7 patients per day
  • Day shift schedule, 5x8 hours, 8:00 AM - 4:30 PM
  • Uses HCHB (Homecare Homebase) EMR
  • Territory covers up to a 60-mile radius from the hub office, with flexibility required
  • No on-call required, though an on-call rotation may be discussed during interview

Requirements

  • Minimum 2 years of recent Physical Therapy experience required
  • Current BLS certification required
  • OASIS experience required for completing Start of Care (SOC) assessments
  • Proficiency in HCHB EMR required
  • Active driver's license and personal transportation required
  • License required if applicable
  • Timely documentation and communication skills required

Additional Information

  • Primary responsibilities include home exercise programs, patient and family education, and orthopedic and neurologic rehabilitation
  • Treats conditions including post-op orthopedic surgeries, spinal conditions, stroke rehabilitation, and neurologic disorders such as Parkinson's and MS
  • Scrubs required in any color; jeans not permitted
  • iPad provided; personal cell phone not required
  • Mileage reimbursed at the IRS standard rate
  • Monday orientation includes billable training modules
  • First-time travelers will not be considered for this role
  • Local candidates accepted at the travel rate
  • Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
  • Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
  • Requested time off (RTO) must be submitted at the time of application
