Description

American Traveler is seeking a Home Health Physical Therapist with 2+ years of recent experience and OASIS proficiency for a day-shift assignment serving a geriatric patient population.

Details

• Home Health setting focused on geriatric patient population
• Caseload of approximately 5-7 patients per day
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• EMR: HCHB (Homecare Homebase) required
• No on-call requirement, though on-call rotation may be discussed during interview
• Territory covers up to a 60-mile radius from the hub office
• Local candidates accepted at the travel rate

Requirements

• Minimum 2 years of recent Physical Therapy experience required
• OASIS experience required for completing Start of Care assessments
• Proficiency in HCHB charting system required
• BLS certification required
• License required if applicable
• Active driver's license and personal transportation required
• Strong documentation and communication skills required

Additional Information

• Main responsibilities include home exercise programs, patient and family education, and treatment of orthopedic and neurologic conditions
• Patient conditions may include post-op orthopedic surgery, spinal conditions, stroke rehabilitation, and neurologic disorders such as Parkinson's disease and multiple sclerosis
• Traveler must be flexible with assigned territory
• Monday orientation includes completion of required training modules
• Dress code is scrubs of any color; jeans not permitted
• iPad provided for documentation; personal cell phone not required for work use
• Mileage reimbursed at the IRS standard rate
• First-time travelers will not be considered for this role
• Travel pairs accepted, though each traveler is assigned their own territory and must have independent transportation
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• RTO requests must be submitted at the time of application
