HonorVet Technologies is seeking a travel Home Health Physical Therapist for a travel job in San Ramon, California.
Job Description & Requirements

Home Health PT - Area #6: Vacaville, Fairfield, Rio Vista, Suisun City, Dixon
Area #7: Vallejo, Benicia, Napa, American Canyon, Yountville
Start Date: ASAP , Compliance takes about 2 weeks
Shift: 8hr Days
Ratios: 1:1 (*May visit up to 6 patients per day; 27 productivity points standard)
Guaranteed Hours - 36 hours weekly But traveler must see 5-6 patients a day. Guaranteed Hours will not be applied in first 2 weeks for ramp, holiday weeks, and time off weeks
First-Timers Accepted: NO
Weekends: Can pick up patients on weekends to meet productivity
Is On-Call REQ?: No
Locals Accepted: Yes
Experience Requirements:
1 YR of Home Health Experience
Case Load: Home Health
Certs REQ: BLS
License: Must have CA license
Additional Notes:
Traveler is expected to have their own transportation.
System does a good job grouping clients minimizing drive and windshield time.
Occasional maximum radius of 50 miles.
Timely communication and documentation required
Documentation tools will be provided, and must be returned.
