Job Summary:
Program Coordinator in a healthcare setting, overseeing the coordination of patient care programs and ensuring the delivery of high-quality services. This role involves collaborating with interdisciplinary teams, supporting patients and families, and managing program logistics to promote effective care delivery.

Job Details:

  • Schedule: Night shifts, 12-hour shifts, minimum of 36 hours per week.
  • Required Qualifications: No minimum years of experience required. Must have certifications in Basic Life Support (BLS) and, if applicable, Advanced Cardiovascular Life Support (ACLS) prior to the first shift.
  • Performs patient assessments within 8 hours of admission to develop individualized care plans and collaborate with patients and families.
  • Provides timely documentation of assessments, interventions, and patient responses in accordance with policy.
  • Participates in the patient rehabilitation program on an assigned unit, utilizing the nursing process to deliver direct patient care and education.
  • Initiates discharge planning and patient teaching from the day of admission.
  • Communicates changes in patient status effectively among care team members to ensure continuity of care.
  • Provides supervision and guidance to ancillary nursing personnel, utilizing problem-solving and organizational skills.
  • Ensures personal and environmental safety following facility policies.
  • Contributes to team building and a positive work environment by maintaining effective communication with management and staff.
  • Assumes charge responsibility following orientation period as requested.
  • Pursues professional development through continuing education and self-study.
