Registered Nurse (RN) – Home Health

Weekly Hour Guarantee: 36 hours Location: Bedford, PA Coverage Areas: Bedford, PA  Mileage Reimbursement: Yes (From first patient to last patient) Shift Schedule: Days, Monday–Friday, 8:00 AM–4:30 PM Dress Code: Scrubs – Royal Blue or Black On‑Call/Weekend Requirements: Yes Pay Rate: $47-$48/hr

About the Role

We are seeking an experienced and highly skilled Home Health Registered Nurse (RN) to provide comprehensive in‑home clinical care. This role requires strong assessment abilities, excellent clinical judgment, and the ability to work independently in a field‑based environment. The RN will complete OASIS assessments, develop individualized care plans, and perform return visits to support patients in safely managing their health at home.

Key Responsibilities

• Clinical Excellence: Provide skilled nursing care in the home setting, including IV therapy, wound care, wound vac management, and venipuncture.
• OASIS Assessments: Complete accurate and compliant OASIS assessments and documentation.
• Care Plan Development: Create individualized, patient‑centered care plans based on clinical findings.
• Return Visits: Conduct follow‑up visits to monitor progress, adjust care plans, and deliver ongoing skilled interventions.
• Education: Teach patients and caregivers about disease processes, medication management, and home‑based care techniques.
• Care Coordination: Communicate with physicians and interdisciplinary team members to ensure cohesive, high‑quality care.
• Field Autonomy: Manage a full‑time field schedule while adapting to patient needs and travel requirements.

Required Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Home Health experience (required)
• Proficiency with:

  • HBHC
  • IV therapy
  • Wound vacs
  • Wound care
  • Venipuncture
  • Patient disease teaching
• Reliable transportation and valid driver’s license
