Physical Therapist (PT) – Home Health

Weekly Hour Guarantee: 32–40 hours Productivity Expectations: 30 points Coverage Areas: York County & Western Adams County Schedule: Monday–Friday, 8:00 AM–4:30 PM On‑Call / Weekend Requirements: None Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient Pay Rate: $49/hr

Position Overview

The Physical Therapist will serve as the primary PT for the agency, providing comprehensive home‑based physical therapy services. This role requires strong clinical judgment, autonomy, and excellent communication skills to manage a full caseload and ensure high‑quality patient care.

Top Responsibilities

• Start of Care Assessments — Complete SOCs, evaluations, reassessments, and discharges
• Plan of Care Development — Establish individualized POCs aligned with patient needs and goals
• Caseload Management — Independently manage and prioritize daily patient visits
• PTA Supervision — Provide oversight and guidance to Physical Therapist Assistants
• Patient Scheduling — Call and schedule patients daily to maintain continuity of care
• Documentation — Complete accurate, timely documentation in accordance with agency and regulatory standards

Required Qualifications

• PT degree from an accredited program
• Active state licensure
• Home health experience preferred
• Valid Physical Therapist (PT) degree and licensure

• Solid‑color scrubs or business casual attire
