Contract (13 Weeks) | Days | Monday–Friday, 8:00 AM–4:30 PM | On-Site | Expected 36 hours/week

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting under the supervision of a licensed Physical Therapist
• Implement and carry out established treatment plans, documenting patient progress and adjusting care as directed
• Utilize EPIC EMR for accurate and timely documentation of patient care activities
• Adhere to all dress code and organizational policies, including wearing black scrubs
• Maintain professional communication with the interdisciplinary care team to support optimal patient outcomes

Required Qualifications

• Active Licensed Physical Therapy Assistant (LPTA) license – Virginia or Compact license required
• BLS certification (American Heart Association only)
• Valid Driver's License required
• Minimum 2 years of Home Health LPTA experience
• Previous experience with EPIC EMR
• No weekend or on-call requirements

Preferred Experience / Skills

• Strong background in home care and community-based therapy settings
• Familiarity with home health regulatory and documentation standards
