Key Responsibilities
Provide physical therapy treatment services to residents under the supervision of a licensed Physical Therapist
Implement individualized treatment plans in a skilled nursing facility setting
Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
Collaborate with the interdisciplinary care team to support resident goals and outcomes
Required Qualifications
Active Physical Therapy Assistant (PTA) license in the state of Wisconsin
Graduate of an accredited Physical Therapist Assistant program
Ability to work a consistent Monday through Friday daytime schedule
Preferred Experience / Skills
Prior experience working in a skilled nursing facility (SNF) or long-term care environment
Familiarity with geriatric patient populations and related treatment approaches
