Contract | Full-Time | Days | Monday – Friday with weekend rotation approximately every 6 weeks
Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting within an assigned territory
- Perform and document patient visits using the HCHB electronic medical records system
- Meet productivity expectations of 28–30 points per week
- Conduct Start of Care assessments as required
- Participate in a weekend rotation schedule as assigned
Required Qualifications
- Active Physical Therapy Assistant (PTA) license
- Prior home health experience required
- Experience with HCHB (Homecare Homebase) EMR system
- Experience with Start of Care documentation
- Ability to travel within an approximate 40-mile radius of the assigned office location
Preferred Experience / Skills
- OASIS experience strongly preferred
- Comfortable working independently in a field-based, patient home environment
