Contract | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Optional weekend on-call rotation (1 weekend per month)
Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting across an assigned geographic territory
- Complete patient visits in accordance with a productivity standard of 6.4 visits per 8-hour day (32 visits per 40-hour week)
- Travel to patient homes within the designated service area surrounding the Torrington region
- Participate in a brief office orientation during the first two weeks of assignment
- Participate in optional weekend on-call rotation, approximately once per month
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in good standing
- Minimum of 1 year of home health experience
- Ability to travel within approximately 40 miles of the Torrington, CT area
- Reliable transportation for daily patient home visits
Preferred Experience / Skills
- Experience with Home Care Home Base (HCHB) electronic medical records system
- Familiarity with the northwest Connecticut region, including surrounding towns
