Key Responsibilities
- Provide direct physical therapy or physical therapy assistant services to residents in a skilled nursing facility setting
- Evaluate, plan, and implement individualized treatment programs to improve patient mobility, strength, and functional independence
- Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Required Qualifications
- Active state licensure as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) in Wisconsin
- Current CPR/BLS certification
- Ability to work day shift hours
Preferred Experience / Skills
- Prior experience working in a skilled nursing facility or long-term care environment
- Familiarity with Medicare documentation and compliance requirements
