Key Responsibilities
- Deliver physical therapy treatment services to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans as established by the supervising Physical Therapist
- Document patient progress, treatment sessions, and clinical outcomes in accordance with facility and regulatory standards
- Communicate effectively with the interdisciplinary care team regarding patient status and progress
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in the state of Minnesota or eligibility to obtain
- Graduate of an accredited Physical Therapist Assistant program
- Experience working in a skilled nursing facility (SNF) setting preferred
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior experience in a SNF or long-term care environment
- Familiarity with electronic medical records (EMR) documentation systems
