Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM
Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting within a 50-mile radius of the office
- Implement and carry out treatment plans established by the supervising Physical Therapist
- Meet productivity expectations of 30 points per week (approximately 6 points per day)
- Accurately document patient care and progress using the HCHB electronic medical records system
- Communicate effectively with the supervising PT and care team regarding patient status and progress
Required Qualifications
- Active Physical Therapist Assistant (PTA) license in the state of Oregon
- Previous home health experience required
- Previous experience with HCHB (Homecare Homebase) required
- Must be able to travel within a 50-mile radius of the office
Preferred Experience / Skills
- Strong time management and self-directed work skills suited for a field-based role
- Experience meeting productivity standards in a home health environment
