Key Responsibilities

• Deliver physical therapy treatments to residents under the supervision of a licensed Physical Therapist
• Implement individualized treatment plans in accordance with established goals and clinical guidelines
• Monitor and document resident progress, reporting changes in status to the supervising Physical Therapist
• Collaborate with interdisciplinary care teams to support resident rehabilitation and functional outcomes
• Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Illinois
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Ability to work effectively in a skilled nursing facility environment

Preferred Experience / Skills

• Prior experience working in a skilled nursing or long-term care setting
• Familiarity with MDS documentation and Medicare guidelines in a SNF environment
• Strong interpersonal and communication skills for working with an elderly population
