    Key Responsibilities
    • Provide physical therapy treatment services to residents under the supervision of a licensed Physical Therapist
    • Support post-hospital, short-term rehabilitation as well as long-term and respite care populations
    • Assist in developing and implementing individualized treatment plans to address residents' rehabilitation needs
    • Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
    • Participate in weekend rotation coverage as scheduled
    Required Qualifications
    • Active Physical Therapy Assistant (PTA) license in the state of New Hampshire
    • Graduate of an accredited Physical Therapist Assistant program
    • Ability to work a Monday-Friday schedule with weekend rotation
    Preferred Experience / Skills
    • Prior experience in a skilled nursing facility or long-term care setting
    • Experience working with Medicare and Medicaid patient populations
    • Familiarity with short-term rehabilitation and post-acute care environments
