Contract | Full-Time | Days | Onsite / Field-Based

• Provide physical therapy assistant services to patients in a home health setting across the greater Sacramento County area
• Treat a variety of diagnoses including cardiopulmonary, orthopedic, neurological, and geriatric conditions
• Maintain a productivity goal of 5–6 patient visits per day
• Deliver care in accordance with the supervising physical therapist's plan of care
• Document patient progress and communicate updates to the care team as required

• Active Physical Therapy Assistant (PTA) license in the state of California
• Ability to work independently in a home health / field-based environment
• Valid driver's license required
• Minimum of two supervisory references from the submitting modality (at least one within the past 12 months and one within the past 3 years)

• Prior experience in a home health setting
• Experience treating cardiopulmonary, orthopedic, neurological, and/or geriatric patient populations
• Established comfort with independent caseload management and field-based scheduling

• Guaranteed 40 hours per week with a consistent and manageable daily caseload
