Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting across an assigned territory in the Portland, OR area
- Document patient care and progress using HCHB (Homecare Homebase) software
- Carry out treatment plans established by the supervising Physical Therapist
- Meet productivity expectations of 27–30 visits for a full-time, 40-hour schedule
- Travel throughout the assigned service territory to deliver patient care
Required Qualifications
- Active PTA license in the state of Oregon (or eligibility)
- Prior Home Health experience required
- Proficiency with HCHB (Homecare Homebase) documentation system required
- Ability to manage a caseload independently across a defined geographic territory
Preferred Experience / Skills
- Familiarity with the SE Portland metro area and surrounding communities
- Strong time management and self-directed scheduling skills
- Experience managing productivity benchmarks in a home health environment
