Key Responsibilities
- Provide physical therapy assistant services to home health patients across Santa Clara County
- Manage a caseload of approximately 25 patients, seeing 5–6 patients per day
- Travel to patient homes throughout the assigned service area; mileage reimbursed at the federal rate
- Document patient care and progress using HCHB (HomeCare HomeBase) software
- Collaborate with the supervising Physical Therapist and interdisciplinary team to support patient goals
Required Qualifications
- Active PTA license in the state of California
- Prior home health experience required
- HCHB (HomeCare HomeBase) experience required
- Valid driver's license and reliable transportation for home visits
Preferred Experience / Skills
- Experience managing a high-volume home health caseload
- Strong time management and independent clinical decision-making skills
