Key Responsibilities
- Provide physical therapy assistant services to patients in a home health setting
- Manage a productivity caseload of approximately 30–34 points per week
- Document patient care and progress using Home Care Home Base (HCHB) electronic documentation system
- Travel to patient homes within the northern region of the service area
Required Qualifications
- Active PTA license in the state of Connecticut
- Prior home health experience required
- Proficiency with Home Care Home Base (HCHB) documentation platform required
- Must reside within approximately 40 miles of the Mansfield, CT office area to be eligible
Preferred Experience / Skills
- Experience managing productivity-based caseloads in a home health environment
- Strong time management and independent work skills suited for field-based care
