Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting
• Manage a caseload of approximately 30–35 visits per week
• Travel to patient homes within the northern region of the assigned service area
• Document patient care using Homecare Homebase (HCHB) electronic medical records
• Collaborate with the supervising Physical Therapist to implement and progress individualized treatment plans

Required Qualifications

• Active PTA license in the state of Connecticut
• Prior home health experience required
• Proficiency with Homecare Homebase (HCHB) required
• No on-call or weekend requirements
• Candidates must reside within 40 miles of the Mansfield, CT office to be eligible

Preferred Experience / Skills

• Experience managing a high-volume home health caseload
• Strong time management and independent scheduling skills
• Familiarity with productivity standards in a home health environment
