Key Responsibilities
Provide home health physical therapy services including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits
Treat approximately 6.5 patients per day across assigned home health territory
Work with patients presenting with diagnoses such as CHF, orthopedic conditions, and pneumonia
Document and manage patient care using Home Care Home Base (HCHB) software
Participate in rotating weekend and holiday coverage as scheduled
Utilize company-provided phone and tablet for field-based documentation and communication
Required Qualifications
Active Physical Therapy Assistant (PTA) state license — must be in hand at time of submission
Prior home health experience required
Proficiency with Home Care Home Base (HCHB) required
Current BLS certification required
Ability to perform SOC, ROC, recertifications, evaluations, and routine visits independently
Preferred Experience / Skills
Experience as a traveling clinician preferred
Candidates with established clinical experience preferred over new graduates
