    Contract | Days | Full-Time | 5 x 8-hour shifts per week | Rotating weekend schedule | Holiday rotation required

    Key Responsibilities
    • Provide home health physical therapy services including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits
    • Treat approximately 6.5 patients per day across assigned home health territory
    • Work with patients presenting with diagnoses such as CHF, orthopedic conditions, and pneumonia
    • Document and manage patient care using Home Care Home Base (HCHB) software
    • Participate in rotating weekend and holiday coverage as scheduled
    • Utilize company-provided phone and tablet for field-based documentation and communication
    Required Qualifications
    • Active Physical Therapy Assistant (PTA) state license — must be in hand at time of submission
    • Prior home health experience required
    • Proficiency with Home Care Home Base (HCHB) required
    • Current BLS certification required
    • Ability to perform SOC, ROC, recertifications, evaluations, and routine visits independently
    Preferred Experience / Skills
    • Experience as a traveling clinician preferred
    • Candidates with established clinical experience preferred over new graduates
