Key Responsibilities
Provide physical therapy assistant services to patients in a home health setting
Complete approximately 30 visits per week within an assigned territory in the Indianapolis area
Document patient care accurately and in a timely manner using HCHB electronic medical records
Collaborate with the supervising Physical Therapist to implement and progress individualized treatment plans
Maintain compliance with home health regulations and organizational policies
Required Qualifications
Active Physical Therapist Assistant (PTA) license in the state of Indiana
Minimum of 12 months of home health experience within the past 3 years
Proficiency with HCHB (HomeCare HomeBase) electronic medical records
Ability to manage a caseload of approximately 30 visits per week
No on-call requirements
Preferred Experience / Skills
Familiarity with Indianapolis-area home health service territories
Strong time management and organizational skills in a field-based care environment
