Contract | Days | 5×8-hour shifts | Expected 40 hours/week | 13-week assignment
Key Responsibilities
- Deliver physical therapy treatment to residents in a skilled nursing facility setting under the supervision of a licensed Physical Therapist
- Implement and progress individualized treatment plans in accordance with physician orders and therapy goals
- Document patient progress, treatment sessions, and clinical notes accurately and in a timely manner
- Be open to floating to other units or locations as operationally needed
- Collaborate with interdisciplinary care teams to support optimal patient outcomes
Required Qualifications
- Active California PTA license required
- Minimum 1 year of SNF experience required
- Ability and willingness to float as needed
- Must meet standard credentialing requirements
Preferred Experience / Skills
- Experience with Casamba documentation software strongly preferred
- Familiarity with skilled nursing facility workflows and productivity standards
