Key Responsibilities
- Deliver physical therapy treatments to residents under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans in accordance with physician orders and therapy goals
- Monitor and document resident progress, reporting changes in condition to the supervising PT
- Assist with functional mobility training, therapeutic exercise, and activities of daily living
- Maintain accurate and timely clinical documentation in compliance with facility and regulatory standards
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in the state of Oregon
- Graduate of an accredited PTA program
- Current CPR/BLS certification
- Ability to work day shift in a skilled nursing facility setting
Preferred Experience / Skills
- Prior experience working in a skilled nursing facility (SNF) environment
- Familiarity with Medicare documentation requirements and MDS processes
- Experience with geriatric patient populations
