Key Responsibilities
- Provide physical therapy treatment services to residents under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans in accordance with physician orders and therapy goals
- Document patient progress, treatment notes, and discharge summaries accurately and in a timely manner
- Collaborate with the interdisciplinary care team to support resident rehabilitation and functional outcomes
- Educate residents and family members on home exercise programs and safe functional activities
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in the state of Massachusetts
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR/BLS certification
- Experience working in a skilled nursing or long-term care setting
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with MDS documentation and Medicare guidelines in a SNF environment
- Strong communication and interpersonal skills within a multidisciplinary team
