Key Responsibilities
- Deliver skilled physical therapy services to adult and geriatric patients in their homes across a rural, geographically diverse service area
- Complete approximately 6 patient visits per day
- Collaborate with physical therapists and an interdisciplinary care team including nursing, occupational therapy, speech therapy, and medical social work
- Support patient recovery from illness, injury, surgery, and chronic disease while promoting independence and preventing hospitalizations
- Document patient care and progress accurately and in a timely manner
Required Qualifications
- Active Virginia PTA license required to be in hand at time of submittal, or proof of compact license
- BLS certification through the American Heart Association (AHA) required
- Specialty experience within the last year required
- One supervisory reference required at time of submittal
- Local candidates are welcome and will be considered at the same base rate
Preferred Experience / Skills
- Prior home health experience, particularly in a rural or community-based setting
- Experience working with geriatric populations and patients managing chronic disease
- Familiarity with electronic health record documentation
