Key Responsibilities

• Deliver physical therapy treatment to patients under the direction and supervision of a licensed Physical Therapist
• Implement and progress individualized treatment plans in accordance with established goals
• Document patient progress, treatment notes, and relevant clinical information accurately and in a timely manner
• Communicate effectively with the interdisciplinary care team regarding patient status and progress
• Maintain a safe and therapeutic environment for patients at all times

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of New Hampshire
• Current CPR/BLS certification
• Valid state or federal issued photo identification required for facility access
• Must wear a photo badge at all times while on site
• Expected 35 hours per week

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Familiarity with electronic medical records (EMR) systems
• Strong interpersonal and communication skills within a multidisciplinary team
