Description

Key Responsibilities
  • Deliver physical therapy treatments to residents in a skilled nursing and assisted/independent living setting under the supervision of a licensed Physical Therapist
  • Implement individualized treatment plans in accordance with physician orders and PT evaluations
  • Monitor and document patient progress, adjusting interventions as directed by the supervising PT
  • Educate patients, families, and care staff on therapeutic exercises and safe functional activities
  • Collaborate with interdisciplinary team members to support resident goals and care plans
Required Qualifications
  • Active Physical Therapy Assistant (PTA) license in the state of California
  • Graduate of an accredited PTA program
  • Current CPR/BLS certification
  • Experience working in a SNF or long-term care environment
Preferred Experience / Skills
  • Experience in assisted living or independent living settings
  • Familiarity with MDS documentation and skilled nursing workflows
  • Strong interpersonal and communication skills within a multidisciplinary team
