Description

Key Responsibilities
  • Conduct home visits for adult patients, including Start of Care and Follow-Up visits, seeing approximately 5-6 patients per day
  • Travel up to 75 miles from the assigned care center to visit patients in their homes
  • Meet weekly productivity expectations of approximately 30-32 points per week (equating to roughly 36 patients per week minimum)
  • Complete all patient documentation within 24 hours of each appointment using HomeCare HomeBase EMR and Outlook
  • Attend periodic meetings at the local clinic office
  • Accept all assigned patients on the daily schedule regardless of geographic distance from previous appointments
Required Qualifications
  • Active, unrestricted Physical Therapy Assistant (PTA) license - must be in hand at time of submission
  • Minimum 1 full year of recent Adult Home Health experience required - first-time home health applicants will not be considered
  • Current CPR/BLS certification required
  • Proficiency in HomeCare HomeBase EMR within the last 6 months (or willingness to complete a retraining course during compliance)
  • Must have a reliable personal vehicle, valid driver's license, and agency-required auto liability insurance
  • Must be willing and able to travel up to 75 miles from the assigned care center; drives between patients may exceed 20 minutes
  • Local candidates are welcome and accepted at the same rate as travelers
Preferred Experience / Skills
  • Experience with OASIS documentation (proficiency within the last 6 months preferred)
  • Familiarity with Start of Care and Follow-Up visit workflows in a home health setting
  • Comfort with flexible scheduling, including potential Baylor shift schedules (4x10s, Friday-Monday) depending on location needs
