Job Summary (List Format):
- Provide skilled physical therapy services under the supervision and direction of a licensed Physical Therapist.
- Implement treatment plans to improve residents mobility, strength, balance, and functional ability.
- Deliver direct therapy services, including therapeutic exercises, gait training, and balance activities.
- Observe and report residents progress and responses to therapy to supervising PT.
- Assist with resident assessments, documentation, and care planning.
- Educate residents and caregivers on exercises and safety techniques for continued progress outside therapy sessions.
- Maintain accurate and timely documentation in electronic medical records.
- Ensure proper use and care of therapy equipment and assistive devices.
- Participate in team meetings and collaborate with interdisciplinary staff to support coordinated care.
- Adhere to state, federal, and facility regulations and maintain professional and ethical standards.
- Work as part of a rehabilitation team in a skilled nursing facility/long-term care environment.
