Assignment Details

Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Guaranteed Hours: 40 per week

Responsibilities

Provide physical therapy services in patients' homes
Implement treatment plans developed by the Physical Therapist
Document patient progress and outcomes
Travel throughout assigned territory to patient homes
Maintain compliance with agency and state requirements
Collaborate with interdisciplinary healthcare teams

Requirements

Active Massachusetts PTA License
Current or previous Home Health experience
Homecare Homebase experience required
Valid driver's license
Reliable transportation and auto insurance
BLS Certification (AHA) by start date
TB test completed within one year of start date
