Job Summary:
The Physical Therapist Home Health is based in East Portland, OR, providing skilled therapy services in a home health setting. This position involves delivering patient-centered care by assessing, planning, and implementing therapy plans to promote recovery and independence for patients in their homes.
Job Details:
- Certifications: BCLS (Basic Cardiac Life Support) required
- Education/Experience: At least 1 year of skilled home health experience required; 1 year of experience preferred
- Location Requirements: Candidates must be local within a 50-75 mile radius of the facility
- Schedule: Dayshift, 8 hours per day, minimum 40 hours per week
- Positions may be eligible for first-time travelers
- Travel Requirements: Occasional maximum radius of roughly 60 miles per day; own transportation required
- Clinical Responsibilities: Providing outpatient, rehabilitation, and skilled nursing services; managing patients with orthopedic, neurological, post-surgical conditions, and more
- Charting System Experience: Required, with familiarity with electronic systems such as EPIC and Rover
- Additional Information: Candidate must be prepared to work up to one shift per pay period when called off is necessary; must be able to work on designated holidays with specific scheduling requests made upfront
- Submittal Requirements: Response to whether the candidate has prior experience at a Providence facility, including location and dates
