Responsibilities
- Assess residents to determine physical therapy needs and develop individualized treatment plans focused on mobility, strength, balance, and functional independence.
- Implement evidence-based PT interventions and monitor progress, adjusting plans as needed.
- Document evaluations, treatment sessions, and progress notes in a timely, compliant manner.
- Collaborate with nursing staff and other rehabilitation professionals to coordinate care and support discharge planning.
- Educate residents and families on exercises, safety strategies, and home exercise programs.
- Maintain safety, infection control, and quality standards during all therapy sessions.
Requirements
- Active Physical Therapist license in New Hampshire or eligibility for NH licensure.
- Ability to work effectively in a skilled nursing care environment and collaborate with an interdisciplinary team.
- Strong written and verbal communication skills for accurate documentation and resident education.
