Responsibilities
- Evaluate residents to determine physical therapy needs and develop individualized plans of care
- Implement therapeutic interventions to improve mobility, strength, balance, and functional ability
- Document evaluations, treatment sessions, progress notes, and discharge summaries in the resident’s health record
- Monitor resident progress and adjust treatment plans based on response and goals
- Collaborate with an interdisciplinary team including nursing, therapy assistants, and case managers to coordinate care
- Educate residents and families on home exercise programs, safety, and activity modification
- Ensure safety, infection control, and ethical standards during all therapy activities
Requirements
- Current Pennsylvania Physical Therapist license or eligibility for PA licensure
- Ability to work in a skilled nursing facility setting
- Willingness to commit to a 14-week travel assignment with guaranteed hours
- Strong communication skills and ability to document care clearly
- Must be able to meet the demands of a dynamic, multidisciplinary rehab environment
