Key Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans
Deliver hands-on therapeutic interventions to residents in a skilled nursing setting
Monitor and document patient progress, adjusting treatment plans as appropriate
Collaborate with interdisciplinary care teams to support patient goals and outcomes
Educate patients and families on home exercise programs and safe functional mobility
Required Qualifications
Active Physical Therapist (PT) license in the state of Massachusetts (or eligibility to obtain)
Graduate of an accredited Physical Therapy program
Ability to work day shift in an onsite skilled nursing facility environment
Preferred Experience / Skills
Prior experience working in a skilled nursing facility or long-term care setting
Familiarity with Medicare documentation and compliance requirements
Experience with restorative and rehabilitative therapy programs
