    Key Responsibilities
    • Provide physical therapy services to patients and clients in a home health setting in accordance with a physician's written plan of care
    • Assess, evaluate, and update patients' therapeutic, rehabilitative, and functional status; participate in the development of the total plan of care
    • Evaluate home environments and assess muscle strength, mobility, gait, and range of motion to determine rehabilitation potential
    • Instruct patients, families, and caregivers in the use and care of therapeutic appliances and communicate patient reactions or condition changes to the attending physician
    • Initiate and direct physical therapy programs; supervise Home Health Aides participating in a patient's rehabilitation regimen
    • Prepare and submit clinical and progress summaries; maintain accurate documentation in the patient's health record in compliance with applicable regulations
    Required Qualifications
    • Active Physical Therapist licensure in the state of California
    • Minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehabilitation setting
    • Current Basic Life Support (BLS) certification
    • Strong oral and written communication skills with solid organizational abilities
    • Knowledge of and compliance with Title 22, Medicare Conditions of Participation, Medi-Cal, OSHA, and other applicable regulations
    Preferred Experience / Skills
    • Prior home health experience with familiarity serving a broad geographic service territory
    • Experience instructing patients in the use of prosthetic devices
    • Familiarity with peer consultation processes and ongoing staff competency assessments
