Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver hands-on therapeutic interventions to improve mobility, strength, and functional independence
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care team to coordinate patient care within the skilled nursing setting
• Educate patients and families on home exercise programs and safe functional techniques

Required Qualifications

• Active Physical Therapist (PT) license in the state of Maryland
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility (SNF) setting
• Familiarity with Medicare documentation and compliance requirements
• Experience with geriatric patient populations
