    Key Responsibilities
    • Evaluate, treat, and develop patient care plans in accordance with Physical Therapy standards of practice for adult and geriatric populations
    • Provide home health services to patients with a variety of orthopedic, neurological, and functional mobility diagnoses
    • Document patient care to meet regulatory and reimbursement requirements using point-of-service laptop and EMR system
    • Meet productivity expectations averaging 4.5 patient visits per day
    • Travel to serve patients across assigned coverage areas, with mileage reimbursed at the government-approved rate
    • Participate in a rotating Saturday schedule approximately once every 6–8 weeks, with a compensatory weekday off provided
    Required Qualifications
    • Bachelor's degree in Physical Therapy required; Master's degree in Physical Therapy preferred
    • Active Physical Therapy licensure in the State of Wisconsin
    • Current AHA or ARC Basic Life Support (BLS) certification
    • Active NPI number required prior to start (candidates must initiate the application process if not yet obtained)
    • Minimum experience in home health settings working with adult and geriatric populations
    • Prior travel therapy experience required
    Preferred Experience / Skills
    • Experience with NetSmart EMR or similar point-of-service documentation platforms
    • Strong adaptability and resilience with the ability to manage change in a dynamic home health environment
    • Outgoing, collaborative interpersonal style well-suited to community-based patient care
