Key Responsibilities
Evaluate, treat, and develop patient care plans in accordance with Physical Therapy standards of practice for adult and geriatric populations
Provide home health services to patients with a variety of orthopedic, neurological, and functional mobility diagnoses
Document patient care to meet regulatory and reimbursement requirements using point-of-service laptop and EMR system
Meet productivity expectations averaging 4.5 patient visits per day
Travel to serve patients across assigned coverage areas, with mileage reimbursed at the government-approved rate
Participate in a rotating Saturday schedule approximately once every 6–8 weeks, with a compensatory weekday off provided
Required Qualifications
Bachelor's degree in Physical Therapy required; Master's degree in Physical Therapy preferred
Active Physical Therapy licensure in the State of Wisconsin
Current AHA or ARC Basic Life Support (BLS) certification
Active NPI number required prior to start (candidates must initiate the application process if not yet obtained)
Minimum experience in home health settings working with adult and geriatric populations
Prior travel therapy experience required
Preferred Experience / Skills
Experience with NetSmart EMR or similar point-of-service documentation platforms
Strong adaptability and resilience with the ability to manage change in a dynamic home health environment
Outgoing, collaborative interpersonal style well-suited to community-based patient care
