    Description

    Key Responsibilities
    • Provide physical therapy evaluations and treatments to residents in a skilled nursing facility setting
    • Deliver post-hospital, short-term rehabilitation services to patients transitioning from acute care
    • Support long-term care and respite care residents with ongoing therapeutic needs
    • Collaborate with interdisciplinary care teams to develop and implement individualized treatment plans
    • Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
    Required Qualifications
    • Active Physical Therapist (PT) license in the state of Maine or eligibility to obtain
    • Graduate of an accredited Physical Therapy program
    • Ability to work a part-time schedule of approximately 30 hours per week
    • Availability for weekend rotation as part of the regular schedule
    Preferred Experience / Skills
    • Prior experience in a skilled nursing, long-term care, or post-acute rehabilitation setting
    • Familiarity with geriatric patient populations and associated rehabilitation needs
    • Experience with short-term rehabilitation and discharge planning
