Key Responsibilities

• Provide physical therapy evaluation and treatment services to residents within a skilled nursing facility setting
• Develop and implement individualized care plans in alignment with facility standards and patient goals
• Deliver compassionate, quality care consistent with the facility's mission of excellence in service
• Collaborate with interdisciplinary team members to support resident rehabilitation and functional outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Jersey
• Graduate of an accredited Physical Therapy program
• Ability to work Monday through Friday with weekends as needed

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Experience with geriatric patient populations
• Strong interpersonal and communication skills with a patient-centered approach
