Key Responsibilities
Evaluate and treat residents within a Continuing Care Retirement Community (CCRC) setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care teams to support resident outcomes
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
Active Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program
Experience providing physical therapy services in a skilled nursing, CCRC, or long-term care environment
Ability to work day shift hours
Preferred Experience / Skills
Prior experience in a CCRC or senior living community setting
Familiarity with geriatric patient populations and related therapy protocols
